The
P & T Committee is formed by the
Director General of DOHMS. It constitutes
various senior physicians of different
medical fields and pharmacists of the
department.

It
creates and maintains the Hospital
Formulary. The DOHMS Formulary is based on
the recommendations of the physicians
after being duly approved by the P & T
Committee. Suggestions to the Committee
are welcome at any time.

Its
important functions include making
policies for medication prescribing,
dispensing, administration and monitoring;
therapeutic protocols and procedures,
review |

and
management of any reported
adverse drug reactions and medication
errors; drug restrictions for the
physicians; resolving problems encountered
by physicians, pharmacists, nurses and
other health care providers, regarding
drug prescribing, indications of drugs,
protocols and criteria of drug use and
finally to promote cost-effective and
rational drug therapy in the department. |

Meetings
are held at regular intervals.
Recommendations for drug inclusions and
deletions, letters to the committee on
various issues and any other matter
brought before the committee are discussed
in detail and decisions made. When
required the matter is forwarded to
Sub-committees, like the Biotechnology
Sub-committee, Antibiotic Sub-committee,
etc. for their recommendations and
sometimes to the Purchasing Departments
and end-users for their feedback.
All decisions are finally approved by the
Director General, DOHMS. |
